Non-traditional? We are a progressive organization, dedicated to helping people - including our staff - be successful! LifeSpring has an opening for a full time Case Manager position for the Permanent Supportive Housing Program in New Albany, Indiana.
This position requires home based and community based services and transporting participants.
The Case Manager will be responsible for providing case management services to individuals and families who have previously been homeless and have a disabling condition.
The case manager will assess the needs of the individual or family and maintain a service plan that includes goals related to overcoming barriers associated with homelessness and learning to become self-sufficient and stably housed.
This position works closely with the Program Manager.
Knowledge of resources, benefits, substance abuse and mental health will be beneficial.
The successful applicant will have a Bachelor's degree in a related field.
This is a grant program funded through HUD and does not have production requirements.
Covid vaccination is required.
